Chicago, IL-Local podiatric physician and President of the Illinois Podiatric Medical Association

Chicago, IL-Local podiatric physician and President of the Illinois Podiatric Medical Association, Dr. Joseph Borreggine, recently spoke to the Illinois State Medical Society's Legal Committee looking for support about possible changes needing to be made to the Medical Corporation Act. Currently, podiatric physicians are not included in the Medical Corporation Act which prevents them from participating in becoming voting members in MD/DO corporate practices or allow them to employ MD/DO's into their own practices. With the steady increase over the years in multi-specialty medical groups, podiatric physicians are lobbying to be included in the Medical Corporation Act legislative language so that they can be included in the practice corporate structure rather than just be employees. Patients find new treatment for flat feet; Podiatrists find new implant effective in curing problems associated with arch problems

Chicago-August 17, 2009--Like many Americans, Plainfield, Ill.-resident Tamara Jack suffered foot pain for several years before taking action. Jack, a 20-year old college student who used to run track in high school, dealt with chronic pain in her foot until it became so unbearable it hurt when she pressed her foot on the gas when driving.

“When I was running, I never was able to give it any rest to heal so it just kept getting worse,” said the University of Phoenix elementary education student. Jack said that she went to see a couple of doctors but her foot never healed. Eventually, she was referred to podiatrists Marlene Reid and her husband Larry Kosova, DPM, in Naperville, Ill.

Jack thought her foot problem was related to a stubborn case of plantar faciitis—a common problem that occurs when the tendon connecting the heal bone to the ball of the foot becomes inflamed. Turns out she had a condition called hyperpronation, or flat feet, which is a condition caused by the hereditary underdevelopment of the ankle bone or heel bone that can often cause a variety of foot and musculoskeletal problems.

According to Dr. Kosova, this condition creates an abnormal range of motion of the anklebone inward, forward or downward. The abnormal motion leads to the closing of the naturally occurring space between the ankle and heel bones. The end result is foot pain that is sometimes hard to diagnose. In the past, doctors would treat this conservatively and hoped it would get batter, or in dire circumstances, partial heel surgery was required.

“Heel surgery can be a radical approach for this as it requires the podiatrist to removed a portion of the heel bone,” Kosova said.

Recently, podiatrists like Dr. Kosova and others are turning to a new approach for chronic and debilitating flat feet called the HyProCure implant/stent system made by Gramedica, Inc., Warren, Mich. With the HyProCure implant, the podiatrist inserts a small screw directly into the collapsed space or hole so when the foot bends or flexes, it doesn’t complete collapse. The implant instantly restores the foot's natural anatomy, and therefore corrects the imbalance propagated throughout the body.

Joseph Borreggine, DPM, from Charleston, Ill. has been using the new implant on several patients with good results thus far. Borregine said that patients with hyperpronation also develop lower leg, back and hip problems because the pain causes them change their gait.

“Most of my patients have suffered greatly with painful flat feet and other lower extremity and back, hip, and sometimes even neck and shoulder problems because of their abnormal gait pattern,” he said. “They have tried many over-the counter and/or prescription orthotic inserts which have not solved their condition, either.”

Louisville, Ill. resident Betty Willison had suffered from a painful foot condition for years before finally getting a recommendation to see Borreggine. Willison, 79, always had high arches but she noticed they were gradually collapsing before the pain began.

“My neighbor’s son recommended Dr. Borreggine and I had the procedure done a few years ago,” she said. “I was up and walking, first with a walker, then a cane, then by myself, in a short period of time.”

According to Borreggine, the procedure is done with a local anesthetic using a small incision.

“My patients are happy to hear that such an important procedure can be done with such a small incision, compared to the more traditional large incision procedures that are much more disabling,” he said.

This procedure is covered under most insurance plans and is quite cost-effective for the patient because of the short out-patient procedure time involved as well as the relatively quick recovery from the procedure, Borreggine said.

Tamara Jack had the procedure done on June 3rd and was back biking and exercising in six weeks.

“The procedure worked very well and now I have the option to use this with my other foot, which I’m having trouble with as well.”

Podiatrist Discusses Flatfoot Surgery

Joseph Borreggine, DPM, a local podiatrist and owner of The Family Foot Care Center in Charleston and Effingham, IL performs a type of surgical flat foot correction known as “subtalar joint arthroereisis,” that focuses on repairing the sinus tarsi, an opening on the outside of the foot between the ankle and heel bone. “This canal collapses in people who have flexible flat feet, and it can cause a number of other foot problems down the road,” he says.

Borreggine uses a HyProCure sinus implant to ensure the sinus tarsi remains open. “By using this small stent, which is the
exact shape of the natural opening, to hold the sinus tarsi open, we can allow for normal joint rotation and proper alignment of the entire leg,” he says. The outpatient procedure requires only a small incision, and patients are able to walk the same day, wearing a post-surgical boot for about a week.

“We follow the procedure with some physical therapy, and many patients don’t even need an orthotic afterward,” he adds. Borreggine says he has performed countless such procedures during the past three years and reports it has provided a permanent solution for people of all ages. “If orthotics haven’t worked, this is an option, and we’re glad to have something like this finally available for our patients.”

APMA Awards Illinois and Virginia for Outstanding PR Efforts
During APMA's 89th Annual House of Delegates, two state component societies received awards for outstanding public relations efforts during the 2008 Elect to Save Your Feet (ESYF) campaign last November. Illinois and Virginia were awarded the honor for the following actions:

ILLINOIS: The Illinois Podiatric Medical Association (IPMA) leveraged Diabetes Awareness Month by planning a statewide hotline event to encourage people with foot-care related questions to call and talk to a podiatrist. A news release was distributed individually to a statewide media list that included daily newspapers, television stations, and radio stations. This received great coverage, including a story that ran in the Chicago Tribune. IPMA proved to be quite hip and tech savvy as they used social media to bolster traditional media outreach. A Facebook event was created, and people were encouraged to use Twitter to "tweet" questions.
